General Information

COSTA serves school bus companies, school districts, and livery companies.

COSTA offers Motor Vehicle Record (MVR) Requests, V Training for New and Renewal drivers, and School Bus & STV DVIR bulk booklet orders.

COSTA holds an Annual Meeting in June and a quarterly Safety Council Meeting with DMV agents for all COSTA members.

All immediate services such as MVR requests, V Training Sessions & Certificates, and DVIR’s are available without membership.

V Training Sessions and DVIR booklet orders are available at a discounted rate for COSTA members.

The Annual Meeting and Safety Council Meetings are for COSTA members only, as well as the Scholarship Application and various hosted events & webinars.

Ask your employer. COSTA members are school bus companies, schools, board of educations, and livery companies.

COSTA membership runs on a Fiscal Year basis: July 1 – June 30.

Driver History Records

Please visit the MVR page to download an MVR request form.

Fill out the digital form in the requested format and submit an electronic attachment to administrator@ctschoolbus.org

COSTA cannot accept a scanned version of the document. The request form must be a digital open copy for the records to be appropriately accessible for the admin.

COSTA runs requests typically on Monday’s & Wednesday’s and returns records within 1-2 days.

Due to the collaboration with the DMV, the rate of return can differ around State & Federal holidays.

Please submit requests in a timely manner and allow enough time for processing.

COSTA will send the records in a secured PDF file through a secure messaging site.

COSTA will send an invite for new applicants to access the secure messaging portal with the PDF file. Once invited, the portal will allow access until the email information changes. The secure portal requests a login every couple of months to ensure security protocols.

V Training & R360

If a driver is looking to obtain a V Endorsement on their license to transport students from home to school, they will need to attend V Endorsement Training to obtain an R360 certificate.  An R360 training certificate is a required document to obtain the V Endorsement.

New drivers must attend the 10-hour training to obtain the R360. The 10-hour training is all four classes. 

Renewal drivers must attend the 6-hour training annually to maintain the validity of the R360 & V Endorsement. The 6-hour training is the last two classes. 

V Endorsement Training Sessions are offered monthly.

Yes. The R360 is valid for 6 months (New Drivers) to 1 year (Renewal Drivers).

No. COSTA offers non-member pricing for all V Endorsement training sessions.

No. COSTA accepts individual drivers. When registering leave the employer section of the form blank.

The trainer will send a signed completed R360 and EpiPen certificate to the mailing address provided on the registration form.

Due to USPS processes, please allow at least 1 week for certificate delivery before requesting an additional copy. Additional copies are sent out physically and digitally under extenuating circumstances.

A driver’s DMV appt date is their responsibility and discretion, please be mindful of the timeframe between completion, certificate delivery, and the DMV appt date. Plan accordingly. 

No. The DMV only accepts original R360 certificates.

Therefore, the R360 certificate is mailed to the participant.

COSTA recommends the driver and employer hold on to a copy of the certificate for filing purposes.

Yes. If a company is looking to train 10 or more participants. Please reach out to express interest, reserve training dates, and submit registration forms for each participant.

COSTA has an in-person trainer that can offer after work & weekend dates either in Bethel, CT or within a 45-minute drive.

All coordination is done through COSTA admin before the training session begins. The training session will not be confirmed until registration forms and payment are received.
